Accessing your medical details

Your medical details hold information about you and your medical history. Your medical details have your GP record, which includes information like any conditions or allergies you have and any medicine you’re taking.

They will also include records from any other NHS service you use, like the hospital, dentist or opticians, and will include letters, test results and appointment notes.

How to get your medical details

Download NHS App or go on Systmonline to access records. You may be required to provide photographic proof to access medical records.

Patients can apply for an online account which will enable them to do the following:

  1. Book appointments (Not yet available)
  2. Order repeat prescriptions
  3. View summary care record
  4. View childhood vaccinations
  5. View test results
  6. View their medical history

Patients aged 15 years and above cannot register electronically for an Online Account because their identity must be checked and verified.

Patients will need to attend the surgery with photographic ID to confirm their identity and bring a completed copy of the below form. This form can also be used for adults who wish to gain access to their children's (below 15 years old) medical records and have parental responsibility. The form can also be used for Patients aged 11 years and above who wish to allow another individual access to their medical records

Patients who already have an Online account and who wish to have detailed coded access to their medical record will still have to complete the application form.

It takes around 10 working days to set up an online account because of the requirement to check patient records before approving the detailed coded access element of the application. On approval patients will be issued with a user name and password.
